Development and preliminary evaluation of the caregiver assistive technology outcome measureINTRODUCTION: Assistive technology is often recommended with the aim of increasing user independence and reducing the burden on informal caregivers. However, until now, there has been no tool to measure the outcomes of this process for caregivers. OBJECTIVES: To describe the development of the Caregiver Assistive Technology Outcome Measure (CATOM), a tool developed to measure the impact of assistive technology interventions on the burden experienced by informal caregivers, and to undertake preliminary evaluation of its psychometric properties. METHODS: Based on an existing conceptual framework, existing measures were reviewed to identify potential items in a preliminary version of the measure. Cognitive interviewing was used to identify items needing clarification. A revised CATOM and manual were then reviewed by clinicians. After revising some items based on the interview findings, the measure was piloted as part of an intervention study examining the impact of assistive technology on the users' informal caregivers (n = 44). RESULTS: Based on a review of 12 existing measures, a 3-part measure was developed and questions were refined based on cognitive interviews with informal caregivers and feedback experienced assistive technology practitioners. For the activity-specific and overall portions of the measure, the 6-week, test-retest intraclass correlations coefficients were 0.88 (95% CI 0.64-0.96) and 0.86 (95% CI 0.60-0.95), respectively. The CATOM data correlated as hypothesized with other measures. Caregivers’ experiences with the selection and use of assistive technologyQualitative data from a mixed-methods clinical trial are used to examine caregivers’ experiences with the selection and use of assistive technology to facilitate care recipients’ independence. Through a thematic analysis of interviews from 27 caregivers, three broad themes were identified. “A partial peace of mind” described the generally positive psychological impacts from assistive technology, mainly reduced stress and a shift in caregiving labour from physical tasks to a monitoring role. “Working together” explored the caregivers’ experiences of receiving assistive technology and the sense of collaboration felt by caregivers during the intervention process. Finally, “Overcoming barriers“ addressed two impediments to accessing assistive technology: lack of funding and appointment wait times for service providers. The findings suggest that assistive technology provision by prescribers plays a beneficial role in the lives of caregivers, but access to such benefits can be hampered by contextual constraints. Co-construction of an Internet-based intervention for older assistive technology users and their family caregivers: Needs identification phasePurpose: Providing care to older adults using assistive technology can be challenging for family caregivers. To inform the development of an Internet-based intervention, this study aimed to identify older assistive technology users and family caregivers’ needs related to assistive technology procurement, and to explore how to offer remote support through an Internet-based intervention.
Methods: Based on an iterative user-centered design approach, 30 semi-structured interviews were conducted with stakeholders about their experiences with assistive technology procurement/attribution and their perceptions of a proposed intervention. A modified content analysis approach was used, with a mix of emerging and a priori concepts.
Results: Participants view assistive technology procurement as an ongoing cyclical process, with potential unmet needs at key moments before and after assistive technology procurement. Assistive technology user-family caregiver dyads needs focus on assistive technology information, access, assistive technology-person-context match, training and support. While participants felt there were benefits to the proposed intervention they also identified potential implementation barriers.
Conclusion: Assistive technology Internet-based interventions dedicated to family caregivers should ensure systematic and tailored follow-up while integrating some form of human support. This study guides the prototype design of the proposed intervention towards a graded support approach, empowering assistive technology users and family caregivers to resolve assistive technology-related challenges. Effects of Unstable Footwear on Stance PatternInadequate safety shoe wearing is thought to be responsible of many musculoskeletal disorders. A
new concept of unstable footwear is proposed to reduce mechanical stresses during work. Major
expectations argue for a better ankle dynamic behavior leading to an increased muscular activity.
The aim of this study was to evaluate the variation of stance characteristics while walking on level
ground as a function of the models of safety shoes. Twenty healthy males (aged 23.6 ± 7) were
asked to make six gait cycles on a walking track while barefoot, shod with low cost safety footwear,
high cost one, and equipped by Masaï Barefoot Technology shoes (the most unstable footwear).
Stance phases (heel strike, flat foot and toe-off) have been normalized as percentages of the maximum
value of ground reaction force recorded during each step. The highest significant percentages
have been observed in Masaï Barefoot Technology shoes condition and only during heel
strike (8% to 10.1% higher) and toe-off (10% to 11.3% higher). In contrast, the temporal durations
of the three stance phases were not significantly different between the four conditions. The
convexity of safety shoe induces less stability increasing the adaptation of the muscle activity of

Longitudinal Outcomes Among Family Caregiver's of Power Mobility UsersObjective
To examine the change over 1 year in the burden, wheelchair skills, social support, social participation, and mental health of family caregivers providing assistance to older adult powered wheelchair users.

Participants (N=35) included family caregivers (mean age ± SD=63.7±10.2y) who provided at least 2 hours of general care per week for a powered wheelchair user.

Main Outcome Measures
The Power Mobility Caregiver Assistive Technology Outcome Measure (frequency of care and subjective burden), the Wheelchair Skills Test Questionnaire for caregivers (wheelchair skills), the Interpersonal Support Evaluation List-6 (social support), the Late-Life Function and Disability Instrument (social participation), the Hospital Anxiety and Depression Scale (mental health). Measures were taken at baseline, 1, 3, 6, and 12 months. Descriptive statistics were calculated, and a linear mixed model was used to assess changes over time in the outcomes.
Results
The results showed that the caregivers helped on average with 3 powered wheelchair-related activities and 10 other caregiving activities. They also experienced moderate subjective burden and social participation and were within the normal range for depression and anxiety. Moreover, those outcomes remained stable over the 1-year study period. However, the wheelchair skills scores showed significant changes over time, as the scores improved during the first 6 months of the study.
Conclusion
Given that previous research indicated that subjective burden tends to decline over time among caregivers, the findings of stability in this study may reflect increasing needs among this population of caregivers, who may benefit from additional support and interventions. Naturalistic study of stop-controlled intersections negotiation and respect for speed limits in drivers aged 65 and overSeveral studies suggest that normal ageing could affect the driving behavior, particularly when negotiating intersections. The purpose of the current naturalistic study was to document the behavior of drivers aged 65 and over at stop-controlled intersections and describe the way they deal with speed limits. Twenty participants were recruited for a 7-day experiment (5 men and 5 women from an urban and a semi-urban area). Displacements of the vehicle and images of the driver and scenery were recorded. The analysis of the stop-controlled intersections (986) showed that older drivers seldom stop completely at intersections. Overall, men showed a minimal speed that exceeded 4.99 km/h for more than 60% of the intersections (38% for the women). The braking strategies did not vary as a function of the area of residence (p > .05). A total of 1 561 speeding events were identified. Overall, speeding was more frequent on roads where the speed is limited to 50 km/h (907 events) (p < .05). Of these events, 213 events showed a speed that was 20 km/h above the speed limit. These observations are important because they show that some driving strategies are commonly used by elderly drivers (rolling stops and speeding). The extent to which caregivers enhance the wheelchair skills capacity and confidence of power wheelchair users: a cross-sectional study.Objective
To test the hypothesis that caregivers enhance the wheelchair skills capacity and confidence of the power wheelchair users to whom they provide assistance, and to describe the nature of that assistance.
Design

Participants (N=152) included caregivers (n=76) and wheelchair users (n=76).

Main Outcome Measures
Version 4.3 of the Wheelchair Skills Test (WST) and the Wheelchair Skills Test-Questionnaire (WST-Q). For each of the 30 individual skills, we recorded data about the wheelchair user alone and in combination (blended) with the caregiver.
Results
The mean total WST capacity scores ± SD for the wheelchair users alone and blended were 78.1%±9.3% and 92.4%±6.1%, respectively, with a mean difference of 14.3%±8.7% (P<.0001). The mean WST-Q capacity scores ± SD were 77.0%±10.6% and 93.2%±6.4%, respectively, with a mean difference of 16.3%±9.8% (P<.0001). The mean WST-Q confidence scores ± SD were 75.5%±12.7% and 92.8%±6.8%, respectively, with a mean difference of 17.5%±11.7% (P<.0001). The mean differences corresponded to relative improvements of 18.3%, 21.0%, and 22.9%, respectively. The nature and benefits of the caregivers' assistance could be summarized in 7 themes (eg, caregiver provides verbal support [cueing, coaching, reporting about the environment]).
Conclusions
Caregivers significantly enhance the wheelchair skills capacity and confidence of the power wheelchair users to whom they provide assistance, and they do so in a variety of ways. These findings have significance for wheelchair skills assessment and training. The positive effects of caregiving for family caregivers of older adults: A scoping reviewThe negative social, psychological, physical health, and employment-related effects of caregiving on caregivers are well documented; however, positive effects have received less attention. A scoping review was conducted to synthesize and thematically analyze the results of 24 studies published between 2000 and 2018 regarding positive effects of family caregiving of older adults. Our analysis revealed that positive effects of caregiving are embedded in complex, interconnected relationships. Identified themes were: 1) In relationship with one’s self (the caregiver) described positive effects such as personal growth; 2) In relationship to the care-recipient described positive effects such as a deepened dyadic relationship, and 3) Relationships with others described positive effects such as those derived from new care-related relationships. Seeing the positive effects of caregiving as relational and in relationship may shape environmental factors, as defined by the International Classification of Functioning, Disability and Health, such as the development of assistive devices and social policy. Towards sustainable family care: Using goals to re-frame the user-centred design of technologies to support carersTechnology has been identified as an important strategy in making caring sustainable. This article takes the design process for carer support technology as a lens on the divergent definitions that are in play when governments, technology developers and carers contemplate ‘sustainability’. We argue that a central impediment to finding a productive point of overlap among the three perspectives is a predominant focus on carers’ needs. We contrast this needs-based approach, and its focus on doing the tasks of care, with a goal-oriented approach focused on being in relationships. Treatment of Shoulder Dislocation Due to Rotator Cuff Syndrome: A Comparative Study Between A Manual Relocating Technique and A Shoulder Rehabilitation DeviceOur objective is to introduce a new device designated to reduce shoulder dislocation (METHOD-2) compared to a manual technique (METHOD-1). Eighteen shoulders in 20 right-handed participants (22±5) were evaluated as nontraumatic posterior instability. Patients recorded scores ranging between 71.6 and 88.42% when performing the Japanese orthopaedic association shoulder scale (JOASS). Participants were divided into two groups of tens. A licensed physical therapist applied METHOD-1 on GROUP-1. As for GROUP-2, they completed two sets of 10 tractions and pulls using METHOD-2. Participants were asked to complete a relocating maneuver (METHOD-1 or METHOD-2) followed by
a pain assessment (Borg-CR10) and a «Self-Assessment Manikin» (SAM). A retest was set 6 to 8 days later. The data collected were abduction, elevation, medial and lateral rotation goniometry, as well as, the scores obtained from the SAM and Borg-CR10 scales. A paired Student-T test was realized in order to compare test and retest results (p<.05). GROUP-1 procured a decrease in Borg-CR10 Scale (0.9) and an increase in the SAM scale (1.5). The gains of amplitude in GROUP-1 participants were significantly higher (p<.05) than those attained in GROUP-2 (p>.05). In conclusion, METHOD-2 had no shoulder relocating effects, but decreased the self-reported pain perception. “Setting the sails”: Stakeholders perceptions of an adapted sailing programThe purpose of this study was to explore the experiences of current and former sailors, staff and volunteers who were involved in an adaptive recreational sailing program and identify the factors that impact their engagement. Using an ethnographic approach, participant observations (n = 20) and qualitative interviews (n = 18) were conducted with sailors, staff and volunteers. We analyzed the data thematically and identified two main themes. 1) “Anchors away: reasons for setting sail,” described the benefits of adaptive sailing including learning opportunities, leaving disability onshore, challenging stigma, building a community and engaging with nature. 2) “Running ashore: challenges with program delivery and logistics,” acknowledged the various issues encountered, including issues around accessibility, equipment, scheduling, safety management, and volunteers/volunteering. The findings emphasize how adaptive sailing offers a multidimensional experience and could reduce prejudices against people with disabilities. Challenges regarding accessibility of facilities and transports as well as volunteers were identified. Using technology to support caregivers of older people with dementiaThis article was undertaken as part of a WP2 KTEE initiative in year 5 on behalf of its four core research projects. We submitted an article for The Conversation Canada, which was subsequently published.
Story pitch: Caregiving takes a significant toll on the nearly half million Canadians caring for a family member or friend with dementia. While research and innovation are essential for effective implementation of the Dementia Strategy for Canada, so too is getting the right balance between ‘tech’ and ‘touch’.
